Nurses and other professionals provide health care for patients of all ages wherever they call home: a house, apartment, group home, senior living facility, and those without a place to call home. The transition from one level of care and one place to another can be challenging for the patient, family, and professionals.

The focus today is on the recipients of care. Presenters will share transitional models of care; transitional care across the lifespan including at the end of life, care for marginalized populations, and risks factors associated with transitions of care. Homecare Pathways to Excellence will be presented by a representative from the American Nurses Credentialing Center.

The focus today is issues that affect the caregiving and caregivers. Presentations include: Cultural humility in home care; aiding transitions of care using telehealth in the home; creating a sustainable interprofessional team in home care; challenges, successes, and opportunities.
